Playing on hard, the AI will declare wars, but will never actually send fleets or troops to attack their enemies. They will attack enemy fleets (mine alone) that venture into their territory, and even rarely will follow retreating enemy fleets one system. I have only noticed this problem with wormhole AI, but I have not encountered any other sort of AI. Here is a copy of the save with the war in question.

Expected: When in a war, the AI would actually attempt to attack the enemy, such as sending fleets to raid mining stations, blockade planets, or even invade planets.

Actual: The AI sits in constant cold war, both sides refusing to attack the other. My allies will follow my fleets sometimes, but will not attack on their own. I have a 4 year old war that my allies started that they refuse to actually attempt to win, nor even to attack their enemy. Their enemy cheerfully is doing the same thing, and will only attack if I send ships into his territory.

Reproduce: Declare war on the AI while playing in hard. Wait. Realize that they will never attack you.

Updated information. This only seems to effect AI with wormhole FTL. I started a galaxy with forced warp, and the AI is willing to take offensive action.
 
yes and no. I've experienced something similar, but not quite the same in my first game. (Yes, hard difficulty 'cause I'm masochistic).

Early game. I declared against a wormhole empire. They were mostly reactive in the beginning, moving their fleets to intercept mine, but not much else.
As soon as they managed to sneak a constructor and build a wormhole station inside my territory, however, they went on a crushing offensive against my capital, destroying...well, everything I had. So pretty much much you'd expect from a competent AI.

That's when it gets weird.

After the destruction, the rest of their fleet just...left. I figured they would go repair, rebuild a little, then come back to bombard and invade. But they didn't.
It took me 10 months to rebuild my spaceport, and even more months to roll out new ships and go destroy the wormhole station. During all that time I was a sitting duck, completely vulnerable, but they never came. They didn't even touch my mining stations.
So either they were incredibly compassionate, or something went wrong with the algorithms...
 
Yeah, I think the combat logic needs some work.
