In the add_pop command, some cultures don't seem to work. As US, for example, "add_pop which = african_minor...." would work, while "add_pop which = yankee..." doesn't. If the which specifies an occupation or religion, there doesn't seem to be a problem.
Also, using add_pop to remove pops (with value = negative number) has a tendency to CTD.

What culture are the POPs it creates?

Just a wild guess - do all the failures for add_pop happen if you try to add completely new pop types to a province. Was wondering if add_pop can only increase a pop that was already there, not create a new category.

If so then adding some zero size pops to some provinces may be the answer.
