If the heir of the king is the ruler of a city in the capital county, when the king dies the capital become the city and not the castle! So the new king is now ruler of the city ("wrong holding type") and ruler of the castle (that is not the capital anymore). To solve the problem you have to give the castle to a vassal, then revoke!. You can imagine the side effect...

(On the game: Roger I king of Sicily with Monreale Castle as capital on Palermo County, William, his heir, major of Mistretta, city on the same county. When Roger dies William became king with Mistretta as capital and Monreale as desmede.)
