At the moment, barons simply don't marry often, and established houses like the Sunglasses and Rambtons (to name a few) go extinct without micro-management. In the vanilla game, it makes sense that every single little noble doesn't marry, because that would quickly result in the game outright crashing, but in Westeros knightly houses are often just as old as their lord's.

While it's not a immediate issue that cannot be solved by the player with too much time on her hands, I do think some sort of event that auto-marries male relatives of baron-tier title holders to a random courtier (or a matri-marrage if the holder only has daughters and no immediate male children) could be useful, to make sure that, say, all the little knightly houses scattered around the seven kingdoms don't go extinct within a few generations.

I agree. ive had many of the knightly houses die out too. sucks especially when they are some of your favorite bannermen
