Never seen this before, what does it mean if there is a bird on a portrait?

It's a stork because the lucky lass is expecting a child. In America (perhaps the a much larger portion of western culture) the stork was often used to explain to children how they and their siblings and friends came into being.

Yeah, the stork is used in the same way in Eastern Europe as well.

Some cultures use stork for a boy and crow for a girl

But the stork is generally widely known

Ok, but why I've never seen this on my character's wife when I'm told she is pregnant?

You don't pay much attention to detail?

That, or you had some graphics mod which disabled the bitmap, or file corruption so it never appeared for you before.
