I am trying to edit the defines file so that the portraits switch to the middle age ones at age 50 then to the old age at 60 but for some reason this does not work. It works with other mods such as HIP but not this. Does anyone have a solution?

I toke a look at define and CK2+ doesn't have the old age portrait line in their define, you have to add that line in, so the old age portrait work
or you could edit the main ck2 define file and change the old age portrait to what age you want it to be.
