I've been working on a mod that has included a small change in the map, this is not the issue. The issue is that I've also added countries and the game seems to find it funny to bounce capitals away from where I've set them.

My question to you is, how do I get the game to leave my damn capitals alone and how do I predefine a capital for a title that has not yet been created so that what it is created the capital moves to that province?

I'm pretty sure capital distribution is hardcoded. Basically, a ruler's capital is the richest (by base tax, unmodified) province within the bounds of his primary title that shares his culture. (I'm not sure if religion plays a role) If the title is not recreatable (ie, not tied to provinces) the ruler's capital will bounce from province to province depending on which is the richest and shares his culture. There's no way to define specific capitals for a title, other than to increase the base tax of the province itself.

The bolded part is probably what is tripping me up, though I know of one at least that moved.
 
I played Bohemia and, after conquering some of Northern Africa, managed to conquer Alexandria (the AI had its regiments far away). My capital stayed put. But after Alexandria changed its religion to Catholic... bam! It moved.

Your capital province will be the richest province of your culture and sharing your religion in your primary title area.
If you do not own any land in your primary title area (or if there is no area, for non-recreatable titles), it will be the richest province of your culture and with your religion.
If no province shares your culture, it will be the richest province of your religion.
If no province shares your religion, it will be the richest province period.
Capital move is nearly instant.

Cheiromancer, in your example, what was your primary title when you moved to Alexandria -- and did you own any land inside your primary title?
 
i think the change is entirely richest... my most recent game saw me playing the Duke of Apulia becoming the King of Naples (easy cheesy, I know) and I announce the title and the capital moves to Palermo, which is Arab and Moslem, while my ruler is Norman and Catholic. The capital didn't move to Napoli, which was catholic and richer than my previous capital of Apulia.
