Flags and Titles mismatch

I have a weird bug where about a third of the flags are matched with the wrong titles. For example the flag for k_scotland is swapped with c_gwent.

Now I haven't modded anything or changed any of the flags gfx nor have I installed the CoAs DLC.

Anyone have a clue why this is happening and how it could be fixed?

You need to delete the C:\Users\....\Documents\Paradox Interactive\Crusader Kings II\gfx folder and then restart the game.
