First of all, I suspect this is working as designed...but it is still kind of odd.
Playing as Scotland, one of my family members (nephew) had a claim on the King of Norway title. I went to war with Norway and won, making him the King of Norway. When I went to war however my nephew was the Baron of a castle in one of my counties. When he become King of Norway he kept this castle and it became part of the Kingdom of Norway. The county itself (held by me) and the other holdings within it (held by other Mayors and Bishops) remained in my kingdom...but the single castle he held became a little patch of Norway within Scotland.
While I can understand the game mechanics behind this it doesn't feel (or look) right. In this specific situation, when going to war to enforce the claim of a family member which will make him leave my realm (i.e. becoming King of a different realm), I would expect to have returned to me the holdings of that family member in my realm. As it stands now it could be a big reason not even to use this casus belli, as installing a family member on a foreign throne could mean losing a (big) part of your own kingdom...depending on what he or she holds there.
