I noticed an issue where my prison was slowly filling up with prisoners sitting in their cells awaiting solitary, but not tagged with TODO: Escort Prisoner to Solitary, and manually selecting a free guard and right clicking them did nothing. I checked all the possible causes -- I have free guards, I have solitary cells, they're accessible -- and finally found the cause when I chanced to watch it happen live.

If a prisoner commits multiple misconducts at once and the offenses are tagged with different punishment types (i.e. the prisoner attacks staff with a weapon, and Attacked Staff carries a penalty of 2 hours in Solitary and Found Weapon carries a penalty of 1 hour in Lockdown), they'll be locked down appropriately and serve their time -- but that's it. They then stay cuffed in their cell forever with a Solitary sentence to be served.

Saving and reloading the game did not clear the issue, nor quitting and restarting. I've attached my save file.

hey i have the same error. I didn't check if it was with a few penalties but it looks the same. A chained prisoner is waiting for escort to solitary confinement or to a cell. The guards are not escorting him. I have that with every map and I've been playing for a week or so. After a long time, prisoners die en masse of starvation
 
One thing I did notice is that my prisoners weren't starving to death; my guards were treating them like they were on permanent lockdown and going to get food for them, then delivering it to their cells.

So this sounds like you two have different problems, would you say? Or at least different manifestations of a problem?

I hate to necro this after five months, but I've got this exact glitch, and it is legit game breaking. My prison is basically stuck in a death loop: the guards won't take anyone to solitary even though I've got six solitary cells sitting empty and a dozen guards doing nothing (they ain't even deployed), so the prisoners just sit in their cells all day getting angrier and angrier and angrier, and if you let them out they'll snap and go murder someone and get thrown back into the cell waiting for solitary.

Nothing fixes this. I've saved and reloaded, I've exited the game and reloaded, I've redesignated the solitary cells and I've even tried to order this around manually - nothing works to fix it. My game's basically over in the first few hours of playing because everyone gets stuck.

I've sent the save. My prison is (or maybe was as it seems the save is legit KIA and can't be fixed by anything I can do) still young, so I've only got the one cell block and a tiny area for the criminally insane, but you can see with both that the behaviors are completely bugged out: people are stuck in their cells and you mouse over them, see the punishments, and see none of them as being sent to solitary. They get stuck in the cells, they get angrier and angrier, and the game's basically been killed by the bug because the whole prison punishment system is broken.

The same thing applies to the criminally insane - they're over in the infirmary because they got into fights, got beat down, ended up there somehow and the guards won't take them to the padded holding cell. I've got plenty of guards (17 for a population of 41 inmates) and it lists 15 of them as available, but they don't do anything to sort this out and checking the jobs page shows no jobs for them to do anything with regard to the inmates.

Funny enough, they still deliver birthday presents and do so the moment they arrive at the door, but other than that, nothing. Whole game is legit ruined by this bug, so I really, really hope it gets a patch - I just bought all the DLC except Undead and Future and find out the game's basically unplayable due to this thing :p

Hey there, after reading your description of the issue I had an idea of what the issue was and after opening the save I've confirmed it.

You have criminally insane prisoners in need of solitary.

While I turned three of your solitary cells into padded I just ended the punishment on all your CI prisoners to speed things along. I also attached your staff room to the kitchen and raised the break percentage as all of your staff were pissed off.

Attached is the save. However, your prison is definitely having some struggles. Putting the CI so far away from the canteen is giving them some trouble. As is the fact you only have one line of fence with Escape Plans turned on.

Wait, wait, wait - how'd that fix the problem with the regular prisoners getting stuck? I had an idea of what was wrong with the CIs, but I'm not sure how that translates to the regular prisoners getting stuck without being sent to solitary. That sounds like the missing piece here for me.

Also, thanks for the advice on the rest of the prison - I'd had some ideas of how to fix the kind of problems that you've listed, but I'd had a really rough start and this problem soaked up a lot of funds so I haven't been able to fix them. My prison will look a lot better and work a lot better when I can get some cash in :p

The guards don't discriminate between CI and Non when it comes to escorting. So they get 'stuck' trying to escort CIs to non-existent places. Then the non prisoners get stuck. Due to how the game was coded originally the devs can't change it.

Ah! That makes sense. Thanks for this - it might not be possible to fix this issue, but might it be possible to put a notification of it somewhere, or an alert? This isn't so much a bug as it is a case of the priorities being weird.

I’ve passed on this thread and that suggestion to the devs. You’re not the first person to have this issue.
