So, here are a few odd, quirky behaviours of criminally insane inmates and escape logic that explains the strange, unexplained escapes that mystify many players, that I personally have witnessed as I watched the entire escape first hand:
1) guards pretend they don't exist when they are in staff-only areas.
2) Orderlies are not called to deal with it when staff witness CI inmates in staff only area, even when orderlies are otherwise available to help.
3) CI inmates wait patiently in front of a jail door located within a staff-only area until a staff member has to cross it, guards included (a guard just happened to have been the one opening the door for him when I noticed).
4) As soon as the CI inmate is out of the jail, he makes a run for it.
My personal theory is that being within a "Staff-only" area without supervision doesn't seem to qualify as an infraction (like "trespassing"), and CI Inmates are the ones whose odd behavior and pathfinding leads to staff-only area even when otherwise not misbehaving, as other inmates seem to respect what "staff only" means unless they're explicitly up to no good. Once the Criminally Insane inmates are close enough to an exit, their aimless wandering turns into an escape attempt, and then and only then is a response called.
