First make sure you have updated your video and audio drivers. The fact that when you replaced your HD the game started to crash. Sort of implies that it is a driver issue. Your fresh install did not have one of the drivers the old install had added.

Is it just this one game that has become corrupt or does this happen everytime you play a game?

I need full system specs please. Also include the publisher version, such as Strategy First, PanVision ... and the patch level you are using.
 
One cause of a corupted game is having a province listed as owned/controled by two different nations.
This can happen when a nation is annexed or inherited. Did anything like that happen in your game?

You may also be able to get passed the date that the game crashes by playing it as a different nation, then after you are passed the date it crashes on switch back to your nation.
