I played as an Irish duke of Connaucht, and renamed it County Mayo. I started a game as Habsburg, and noticed that it was still named Mayo. I'd have thought it was one-game only.

I also noticed that I could not re-name any of my holdings, even the newly-built castles.

By the way, when I started another new game, and noticed that Connacht was named Connacht, and I can once again rename provinces.

To me, at least, this implies a good chance that there's a relationship between a prior name carrying over and the inability to name holdings.
 
Did you load your previous game when you started your game as Habsburg?

It's possible that I finished my Ireland game and started Habsburg without restarting CK2, but it's been some time and I cannot recall with any certainty. I'd say it was likely, as Habsburg was my next game, and Ireland I saved and walked away as I'd gotten myself into a real mess (two crowns, Greek Queen)(I need an "only my culture" option for guardian selection). I'll see if I can't play around with it some tonight when I get home from work have some time.
