Please explain your issue is in as much detail as possible.
This has happened to me several times now. While playing a normal game (not ironman) I get dragged into a war by a faction with which I have no diplomatic ties whatsoever against a faction with which I also have no ties whatsoever, not even rivalry. I simply get a popup telling me that I have accepted to join a war against such and such. And I have been playing Stellaris long enough to know that I did not make any rookie mistakes as missing a popup or accidentally supporting independence or any such similar thing. I just get two popups simultaneously: one telling me that I accepted the invitation to war and another that I am at war. I've seen that other players have had the same bug since the middle of last year and it is about time that you solve it as it makes the game utterly unplayable and unenjoyable.

Steps to reproduce the issue.
It doesn't matter if I do anything differently, even when I went back to an earlier save, the war still gets declared on the same date.

This has happened to me too, in one game twice. I quit it after the second time because it was just no fun to fight somebody else's war. I tried to play through but it dragged on and on for decades because I refused to free up my "ally's" systems.

I too received no notifications and don't understand why this is happening. I have over 1600 hours into the game. Please, please fix this. I am playing Ironman, fwiw.
