Please explain your issue is in as much detail as possible.
When a colonist dies by some reason, the "note" on the left keep counting hundreds of deaths (but no more colonist dies). The voice actor starts saying "a colonist has died" multiple times... it never ends and the counter stops at 4 dead.

Can you replicate the issue?
Every time...

Having a similar bug

I have been having the same issue. The first time it happened I restarted from a auto save about 15 min prior and was able to play for a few more hours then it happened again.
