The barren trait currently gives -50% fertility and is visible from birth. I'd suggest to either make it -100%, as a condition causing infertility that would have been visible to people in the middle ages (such as a severe malformation of the reproductive system or a complete absence of menstruation) would very likely have made the woman completely infertile, or turn it into a hidden trait that only becomes visible after several years of marriage. In that case the trait wouldn't signify a "medical diagnosis", but rather the perception of others that a wife who hasn't become pregnant after 3 or 5 years must be barren. It could also remain a -50% fertility modifier then as the problem might just as well be on the husband's side and the woman wouldn't necessarily be completely infertile.
