As King of Sicily in the hundred years war scenario I was able to become both king of Cyprus and Tunisia. While the system counts my prestige well, taking into account the 4 dukes under my rule in order to give me prestige, it does not count me as a King for the size of my realm. For the system I'm a duke and thus can't get the x2 realm size boost of the kings. Since I got 3 king's titles I think it a bit weird.

That is because they, Sicily and Cyprus, are not real kingtitles, they are 'upped-up' duchies, who are just made 'kingdoms' in that specific scenario.

Only Tunisia is a 'real' kingdom, meaning that has the same bonuses as a kingdom.
 
That much I had guessed, but I don't understand why, even after save and reload, the system does not give me the bonus from my King of Tunisia title... Bah, I'll try to conquer some more lands ( Napoly ? Italy ? ) and see if it changes anything...

That won't bring anything. The first title is what counts and that says you are King of Sicily although that is only a Duchy. So you won't get the bonus until you change your primary title. The fastest way is to give your son Tunisia and then play as him. Then, when the old King dies, you will be primary King of Tunisia and then 'King' of Sicily.
