Quick Question:
Did you land your Heir?
When yes, this is the issue, never land your Heir. Never.

And why, someone should land the own Heir, their is nearly 0 reasons in doing it, because your Heir will never be able to get the Physician Trait, if you land him, if you don't land him, he will be able to get this Trait and a lot more, because your Heir nearly always, will travel with you.

I did not land my heir. I never land my heir nor any of my children.

The disappearing Accolade was held by one of my other children. After the Accolade vanished he went back to being a regular knight.

My heir actually held another Accolade. Upon succession he vacated his Accolade as expected and it went to the Accolade's successor.
