Does this only happen in MP? If it also happens in a normal game, you can probably fix it by deleting the "flags" folder in "Documents/Paradox Interactive/Europa Universalis IV/gfx" (not the folder where eu4 is installed). This should make eu4 recalculate the flag files on the next startup
