I've had the same problem but on a different map, and furthermore when I did the same map on a different mission, the ladder worked.

Also experienced this bug after saving one evening mid tactical and loading back in the next day. Thought it was due to a save/load not recognising the ladder had already been triggered.
This was on a Casablanca style map and not the one you have pictured, but the one you have seems to often spawn an item inside of the cliff face just down the ladder that seems to be unreachable.
