Every time I buy a slave camp level or take slaves in a raid the camp loses all of it levels within a few days. This didn't start occurring until 100+ years into the game and has been happening for about 10 years now across two rulers and a title change.

I've seen this when I played a patrician in Myr however as the Pirate King the slave camp stayed where they where.

Playing as Pirate King, happened to me once after about 120 successful years of human trafficking.

edit
Every time I gain a new camp level it will disapear shortly thereafter.

That's... interesting. What's the current slavery law in your capital's crown-law title?

Something is for sure bugged with that. I formed Giscar as Dany, slavery of course being illegal, and my vassals could still go back to slavery, and did so after some years.

Year 8130 at after a 8000 start

1.) Slavery level for my capital's crown-law title is "Legal"
2.) In addition to the slave camp disappearing I am unable to sell or buy any new slaves.
3.) Checked through every count and above character in Essos- seemingly only Pentos, Myr and Qarth allow slavery
4.) I did not see a single character with the slave trader trait
5.) Only 9 of those characters had the Slave Owner Trait
6.) Of those 9 characters, only one has a Slave Camp at level one.

7.) Also the King of Voltanis' portrait depicts a ruin

The inability to buy or sell any new slaves is probably linked to the lack of slave traders to buy them from (which, in turn, is caused by one of the AI's peculiar bouts of stubbornness - I thought we'd finally got them to actually engage in the slave trade, but apparently we only managed to get them to do it for a century or so).

I'm still not sure what's causing the slave camps to disappear. Did you get any events when they did? Or did they just vanish?

No event, they just keep vanishing

Pretty much this, however as a Pirate King I was able to keep a rather large slave camp until I was overthrown 250 years later. (the 40k pirate revolt was insane)
But as a low level patrician they disappeared almost immediately

I have a couple of things that seem to be causing the issue. When you are trying to sell a slave, if you can't find a buyer for one, you can lose slave trader and also your slave camp. It happens when you keep clicking through and no one will make an offer. The game also has a tendency on reload to drop the slave trader trait and the slave camps reset to tiny.

Hopefully I'm not necroing this too badly but I am having this exact same problem with slave camps disappearing for no reason. It only began happening recently and now a camp level disappears every few months no matter what I do; I went from extensive slave economy to no slave camp within 2 years without a single prompt, notice, event, etc. No idea why its happening.

I uploaded a copy of my save in case you still needed it below.

I have this bug as well - slave camps have never worked for me. If I buy them or end a slave raid war with the game paused, the camp doesn't appear in my capital at all. If I do it with the game unpaused, it appears then vanishes after a few days with no message. If I start as Aegon in the first or second bookmarks, cheat to add gold, wait a short time, then try to buy a camp, I experience this bug. Is this repeatable for anyone else?
