Please ignore my earlier thread on Holy wars.

Whenever you build / take over a new temple and create a new vassal from them, your current heir becomes the heir to that temple thus divesting them from the line of succession.

Reloading your save after you have created the vassals seems to fix the problem.

This is under Patch 1.4, easily to replicate.

Thank you Paradox for reading / investigating your forums.

It becomes investiture and if my heir was heir to that temple then he/she isn't anymore when creating a new vassal. At least that's what happened for me. Do you have any more information on the issue?

Have the same problem. Was playing as Aragon, declared a holy war on OPM neighbor, got all his holdings, immeadiately created new vassals for town and a bishopic. On the next day my daughter (only child) is not a heir any more. I saved a game, reloaded and my daughter is a heir again.

Ok, will try load the save and see if the heir changes.

UPDATE: Nope, the heir does not change on load for me.

Heir changes not when you load game. It changes when you win holy war.

Example.
Before holy war my heir is my son A. Son B is second in line.
I win a holy war and get a county with castle, city and bishopic. Without unpausing the game I grant city and bishopic to generated vassals.
I unpause the game and on the next day my heir is son B. - Bug is here!!!
If we save/load, our heir will be son A again. - It`s not a bug!

Steps to reproduce:

Start Stamford Bridge scenario as King of Castille. Your heir is King Alfonso VI of Leon. Second in line is King Garcia II of Galicia.
On day 1 declare Holy War for Aragon on Zaragosa.
Win the war.
Create vassals for city and bishopic.
On the next day your heir is King Garcia II of Galicia.

Will look into it further with this updated information. Thank you for following up!

UPDATE: It doesn't work for me, I mean, the steps to reproduce. The inheritance works just fine even when making peace with the Muslims.

I have this problem as well. Started as Leon, assasinated my way to the kingdoms of Galicia and Castille and started conquering the Moors. I created a bishop for my newly conquered church and the next day my heir changed. I was terrified, because the new false heir was horrible. It seemed to reset to the correct heir when some random courtier died, though.

As I conquered more land from holy wars I encountered the problem every time transfer of churches were involved. My character actually died once with the wrong heir shown, but to my great relief the correct heir got the throne anyway. It's still more than just a visual issue though, as your relations with your heirs and pretenders change because of this.
