I have every single gameplay DLC, so it's not that. I'm playing starting from the earliest possible date, it's 8014, and I'm playing as Ambrode Harlaw, a duke on the Iron Islands. Every time I get an event where I get the choice between women to take as a salt wife, no matter who I choose, she remains where she is. Ambrode is married and has two salt wives out of a maximum of 20. He's got the Reaver title (though he wasn't reaving at the tiem, just manually raiding), he's not incapable or infirm or anything, either. He's not a part of his liege's council. I can still turn slaves caught in raids into salt wives, though.

EDIT: Got another Saltwife event, but instead of getting none at all, I got one that I didn't actually choose. Event gave me the wrong lady. This is confusing. (extra edit: ) When it gives me multiple choices, it now always gives me the first choice, no matter what I choose. So instead of getting no saltwives at all, I'm just not getting to pick which one.
EDIT 2: If you're given a saltwife through a reaving event, it works. Otherwise, still busted.

I saved over it to play House Hoare instead. I've been trying to reproduce it since then but it hasn't shown up again, at all. If I run into it again, I'll grab the save and resurrect this thread.
