First of all thank you all for reporting this bug and sorry for not getting a response to you sooner.

I agree that this is a serious issue. Lets get it fixed!

There is currently a known issue where loading an iron man save after starting a new non iron man game will result in a warning that achievements are disabled (due to not being in iron man mode). However, if this warning is ignored achievements can still be earned. This was verified by me and another tester separately today.

From your posts (and from other a few other threads like this one) I get the impression that there is a related issue with the game rules that can in fact stop you from earning achievements. Currently investigating how to reproduce this!
 
Sorry, no news yet. I did try to reproduce this myself last week but couldn't reliably do so following the steps in the OP (I think I succeeded on my first try but weren't able to confirm that I did). Will try again from the start some time this week but if either of you have any additional information on exactly when this issue occurs that would be very helpful!
 
All I have to do is start a new game, Ironman or not. Save it. And when I reload the save, either from the launcher or the main menu, and all the game rules are changed. I've tried it with compressed and uncompressed saves. Custom name save, the generic save name. Nothing seems to work. Like I've said even after editing the save to make sure the game rules are set how I want them, as soon as I reload they always revert.

Is that regardless of what settings you are using? Does it happen even if you use the default rules with 1-2 rules changed at random? Thinking this might be related to either one of the settings in particular (or a category of settings), or to the exact sequence of starting new games, changing rules, saving and reloading.

Did two quick tests now using ONDO 3.0.1.1 where I changed a number of rules (epidemics, Mongol and Aztec invasions etc.) and took note of what settings I chose, let the game play for a little bit, saved, quit the game, restarted, loaded the save and checked the settings. None of them had changed.

To be clear I agree that there seems to be some kind of issue with game rules either changing or being displayed incorrectly. The question is just how to reliably reproduce it so the cause can be found and fixed. Will set aside some time to try one more time following the exact steps of the OP some time this week!
 
As @Corvexi_IX pointed out removing the line alternate_start='yes' from the rules in documents/ParadoxInteractive/Crusader Kings II/interface/game_rules.txt seems to work as a workaround. I think removing it from the first set (there are four sets in the file corresponding to the four rule presets in the game) should be enough to stop this bug from occurring but removing it from the other sets is probably a good idea too. It's also possible that removing the file entirely will also work as a workaround.

With this information we also have a little better understanding of the cause so hopefully we will be able to provide a proper fix sometime soon.
