When a bishop converts religion, they are instantly replaced by a bishop of your religion and culture. I had often seen my message queue get bombarded by bishoprics being usurped when part of the Byzantine Empire, likely due to the emperor demanding conversions to/from Iconoclast
Steps to reproduce the issue.
-Own a bishopric with wrong religion
-Use your court chaplain to convert the bishop, or use diplomacy to demand religious conversion
-Observe as a new bishop steps in for whatever reason
