Please explain your issue is in as much detail as possible.
A while back I reformed the Slavic Church and made sure the ruler would get the title of Velky Volkov, of course. The emperor of The Slavic Union also controls the Slavic Church.

Centuries later, I discovered that the title had somehow been "granted" from myself to my brother about then years prior to noticing. There's just one problem: I absolutely, 100000% did NOT grant ANYONE that title. There's no way I ever would, and I certainly didn't give it to him. This is a bug. A BIG one.

But it gets better. What do you do when a sibling gets a title? Sharpen a few knives and risk that kinslayer malus, of course. So I do. And it gets passed to another relative. And another. And another. And another. And then I realize that all of the new holders who "inherited" that title have no parents, siblings, friends, or anything in those tabs. They are from a specific house, but do not have any links in the family tree at all. Instead of passing the title back to the emperor, the game seems to be auto-generating characters (kinsmen and kinswomen, no less) that get the title. As such, I have absolutely no way from recovering this bug and seem to have uncovered a second one.

Steps to reproduce the issue.
-Open attached game file.
-Check The Slavic Church title and history. See the "granted" status from the current character to a brother.
-Murder the current holder. Shouldn't take long at all.
-Watch it go to a character that didn't exist prior to the assassination instead of back to the player character.

I'm getting this when opening your save file:

I'm running 3.3.2 (IIVC) with no mods, so are you *sure* that the issue couldn't possibly becaused by loading a save started in a previous version of CK2?

I started this game a while back, so it has spanned multiple versions, but that's certainly not an uncommon use case. It opens without any kind of warning on my own 3.3.2 IIVC version and has for some time, so I'm not sure why you're seeing it. Perhaps I should note that I'm running this in MacOS 10.14.6, so maybe there's some kind of issue going cross-platform. I also neglected to note that I have all DLC installed, but Conclave is not active because screw the constant council factions.
 
Well...depending on doctrines on reform, the religion head title could always follow its own succession.
Under some circumstances, the title goes "Open elective" and you're unable to hold it on inheritance.
If your primary(player) heir is not a valid religious heir, the title shows unknown character on succession tab.
And shall this situation happens, it spawn a dynasty member of your current character as hold, with no parents.
A bug? I would say Yes. But the dynasty, not the succession.
 
That's not the case here. My character inherited the title along with all others upon his successor's death. The Slavic Church wasn't "granted" to his brother until five years later. My character is absolutely a valid holder of this title. There has been no event or other disqualifying change that would cause this. Heck, his father was a baby-eating Satanist psychopath and that didn't disqualify him. It's the like the game arbitrarily decided that my character suddenly couldn't hold the title and automatically passed it off with no warning whatsoever.

The worst part about this is that it has completely ruined a 500 year iron man game.
