All of the above
Do you have mods enabled?
No
Please explain your issue is in as much detail as possible.
If you are the de jure vassal of a title that uses Feudal Elective succession but is currently destroyed (i.e. unheld), you will still be able to go to the voting screen in the laws tab (although nominating a successor doesn't do anything), and you'll on occasion get the notification to nominate a successor for the non-existent title.
Steps to reproduce the issue.
This issue can be easily reproduced by creating a title, switching to Feudal Elective, and then destroying it and playing as one of its de jure vassals. In the attached save this situation has been set up with the King of France, where e_france was previously created, switched to Feudal Elective succession, and then destroyed. The notification should appear within a few months of playing.
