Please explain your issue is in as much detail as possible.
I played my game using all mods from the screenshot until 1453. I tried to convert my savegame. The result was a folder with a mod inside Personal Castles folder, but no .eu4 save game.
I tried to start again and play the game just with CK2+, trying to convert a save game in 940-ish. The same result - mod folder and file, no .eu4 save game.
I started a vanilla game. Tried to convert save in 700's with the same result.

Steps to reproduce the issue.
No matter if playing with old mods, updated mods or vanilla - converter is not creating EU4 save game file for me.

I had the same issue. The converter does not generate a save game, it generates a mod for EU4. Start EU4 with that mod and it will start from your save. Very clumsy and poorly explained - I only managed to do it because somebody else told me how.
