Please explain your issue is in as much detail as possible.
What I think : this situation happened due to having the rule "De Jure Requirement" set on REQUIRED.
I became Pontifex Maximus of the Reformed Hellenic religion.
Upon this I aquired the title "Collegium Pontificum", with those nice golden laurels on a purple background. I like this title very much and decide to make it my primary title.
I also got the opportunity, given my prestige and my wealth, to create a new empire. Doing so, I obtained the titular "Hellenic Empire" (custom name).
It backfired though, since my primary kingdom title had no de jure territory. A mere 22 years after its creation, the automatic event triggered, stripping me of my only Emperor title!
I will continue my game and reconquer it, but it was really infuriating to experience.
Steps to reproduce the issue.
1. Play with the rule "De Jure Requirement" set on REQUIRED.
2. Acquire a kingdom rank titular title.
3. Set said titular title as your primary.
4. Create a new Empire through decision.
5. Wait approximately twenty years to see everything crumble in one event.
