Please explain your issue is in as much detail as possible.
The epidemics flag won't show up for you if you are not the doge of a republic, even if the epidemics have spread to the capital province of the republic.
Steps to reproduce the issue.
Play in a republic but not as the doge, wait until an epidemic comes into town and switch between the epidemic mapmode and another one. The flag won't appear though you have a high chance of getting the disease without closing the gates.
