Please explain the issue you experienced in the most condensed way possible
The game shows an adult is still my ward if you look on my character, but on her character she has no tutor.

Please explain how to reproduce the issue


Is there anything else you think could help us identify/replicate the issue?

Quitting to desktop and reloading does not fix the issue.
 
There is a second bug, also linked to education (are both bugs connected?), on your screenshot: your ward still has her childhood trait instead of a level of education.

In my current playthrough, one of my past ward/vassal also never graduated (childhood trait as adult), but I didn't check if he was still my active ward in my (since dead) character view.
I just did a character search on the whole game population to check if any adults had childhood trait, and he was the only one.

I just did a character search on the whole game population to check if any adults had childhood trait, and he was the only one.
That... is extremely weird... and should not be possible. Childhood traits have a "maximum age" limit of 15.
I have just tested this (using cheats), and the traits automatically disappear when the character reaches 16. It also becomes impossible to add those traits then.

A mod could definitely change that though.
 
I am also having this issue now as of today. First appeared yesterday. Disabled all mods, still occurring. I have a 17yo ruler ward and never got the witchcraft conversion event. I had asked my vassal to let me educate him at 14yo so I could convert him.

I also noticed something else odd with age in my game. When my current ruler inherited, he was 16yo and at 17 he got an event and became a 6yo's bully. I thought it was pretty peculiar to see a childhood event. I didn't disable mods for that one to check, but none of my mods have been updated since my previous 2 rulers and I saw no odd behaviors before. This is on the latest patch, only yesterday, with the Persia expansion installed.

Mods running are only QoL mods and should not affect anything in the main game. I have an education reminder mod for close family but other than it is mostly UI enhancement, character search, and the ones that have your council auto convert/auto control/auto develop.
