Please explain the issue you experienced in the most condensed way possible
I started noticing this in the 1.3.x series, so I think it predates Azure, but in a nutshell sometimes I will conquer a large area like a duchy, and then a few months or year later, some of or all of the conquered land will be mysteriously given to another ruler and made independent. I will get a message saying "you are no longer the holder of ....", but no other explanation, and no rebellion was kicked off to trigger it. In one case it was the Duchy of Medina, which I had to reconquer a few times and finally the last time it hasn't happened since. It also occurred with the Duchy of Cordoba, which was adjacent to the Duchy of Toledo that I had also previously conquered. Shortly after conquering Cordoba, most of Toledo suddenly was taken away and given to a new independent muslim ruler, without notice. After I reconquered it, the same thing happens to Cordoba. Eventually, after reconquering Cordoba, both of these were suddenly taken away and given to a new ruler without warning or rebellion war.

Please explain how to reproduce the issue
I am not sure how to reproduce, as it appears to be sporadic, and I have played through many times without this occurring. It seems to occur on my most recent game, and the only way I can think to reproduce is to conquer a Duchy. The only consistency I can think of is both were Muslim territories. However, I played a long way through as Hausa conquering most of Africa without running into this before recently.

Is there anything else you think could help us identify/replicate the issue?
Both times I saw it, it seemed to be on conquered Muslim land - but I've also conquered a lot of other Muslim lands without this happening. I am right now playing a kingdom that is vassal to an empire that I used to head, but now is headed by an AI that's of my dynasty. Again, have no idea if that matters. As well, this save seems to exhibit the same "no garrisons" behavior reportedly introduced, according to other bug reports, by the Azure update.

So I read the explanation in "CK III - Populist uprising steals half your realm and creates new kingdoms without du jour" which might shed a little more light on the issue. Is it possible that my counties' peasants threatened my liege (instead of me) with rebellion, and the AI just gave it to them unchallenged instead of starting a war? I know the county control level in these was low (so I planned to use levies/mercs to put down rebellion if needed). That would also explain why it only occurred to recently conquered land. If so, I feel like that's somewhat unfair as I had more than enough levies to quell the rebellion and as well should have been the target of the rebellion as their immediate liege or some sort of option to intervene.
