Please explain the issue you experienced in the most condensed way possible
In 1066 start, the holder of the Duchy of Aksum (d_aksum) is Mesfin Dawat II. He is independent. He has 3 counties: Dambiya, Begemder, and Semien. None of those counties are in Aksum. They're all in the Duchy of Gondar (d_gondar).
His neighbor is Yemrehana Krestos, who has the the d_gondar title and is also independent. However that character has none of the d_gondar titles; he has one of the counties that is actually in Aksum. His vassal has some of the other Aksum counties.
This doesn't really make any sense; Mesfin Dawat should lose d_aksum and gain d_gondar instead. Meanwhile Yemrehana Krestos should lose d_gondar and gain d_aksum.
Please explain how to reproduce the issue
Load up the 1066 start and look at East Africa

two screenshots sum it nicely. One is Duchy Titles mapmode, the other is Realms map mode. As you can see the Gondar and Aksum duchy titles are just swapped.
