Please explain the issue you experienced in the most condensed way possible
An Iberian culture (Visigothic) is missing access to reform/accept a regional tradition (visigothic codes). While this isn't usually an issue after it fractures, it is very annoying for my playthrough in which I started as Haestine with the explicit goal of preventing the cultural fracture.

Please explain how to reproduce the issue
Load the game as a Vsigothic character and take a look

Is there anything else you think could help us identify/replicate the issue?

To add, the Wiki is also incorrect on this as it says "Visigothic (or descended from Visigothic) cultures"

Oh yeah, it only checks parent cultures:
