This appears to be a known bug on PC, just confirming that it is also happening on console (XB One). Basically the game thinks colonists are outside the dome when they aren't, and they die even though their dome is well supplied with O2.

I'm losing colonists around 10 at a time. Possibly related to working a reactor shift. This is a pretty frustrating bug. I had to walk away tonight.

Notice in the above picture, the suffocating colonists are wearing their space suits while everyone else in the dome is not. I think this group just came in from working outside the dome.

Except colonists remove their space suits as soon as they enter the airlock to the dome, so them wearing their suits while inside the dome means something is definitely bugged and not working correctly
 
I figured out the cause. Building an outdome work building with its exit too close to the dome seems to be the problem. Leave at least one space between the door and the dome to solve the problem.

I guess they would exit the work building inside the dome, then not register as inside the dome because they never used an airlock.
