The province population must be lower than 5000.
You have to send a colonist in the province (it is viewed as a missionary). If he is succesful, the province becomes of your state religion.
Random events can convert provinces' religion too.
No, you cannot convert when pop is 5000 or more.
Yes you can lower population of a province by besieging it. Lower tolerance towards the religion. Odds are revolts will occur. Let the rebels capture the city then smash them and besiege the city. After a few tries, the pop will be lower than 5000...
