Because there's a bug somewhere. It's hard to tell exactly why without using a debugger, so just be glad that it's crashing when it doesn't do any harm. (That's probably why Paradox never fixed it.)
Is it because you use a reserved tag as player? It will be always the case then because reserved tags were never intended to be played, even if some mods propose it for hands-off games.
I've experienced the same thing on occasion, so I doubt it's a version problem.
I have this problem only when the whole world or at least the most provinces are discovered.
