I was an illuminated technocracy and had a choice of 4 leaders in an election, but I didn't have the influence to pick one so I let it decide for itself. One of the candidates died during the election and no-one replaced him. This led to this empty slot being elected, leading to no overseer. I solved it with an emergency election once I had the influence, so fortunately it wasn't too serious! But I presume the problem is that no-one is selected to replace dead candidates/they are not removed from the running.
