We're on xbox and recently almost every time we start up a multiplayer game, it duplicates one of our custom empires as an AI empire. We've tried to change all sorts of settings around and nothing seems to fix it.

We have updated our Community Code of Conduct. Please read through the new rules for the forum that are an integral part of Paradox Interactive’s User Agreement.
This might be the same issue that is causing players to have empires reappear after they are deleted
You have to go into the xbox memory management and delete the empires [this may cause saved games to be deleted as well]
This is likely a possible issue with the cloud
If that doesn't fix it you can try the Xbox Dedicated List of Fixing Stuff (i.e. what Xbox tech support would tell you to do)

- uninstall the problematic games/dlcs
- remove your account from the xbox
- reset your alternate mac address [this will force a system restart]
- once your system has booted back up, hard restart your system [hold the power button for 10s until it is completely off]
- unplug your system
- unplug your internet modem/router [if this step is not doable skip to step 7]
- wait a few minutes
- plug your internet modem/router back in [ignore if step 6 was skipped]
- plug your system back in
- turn your system back on
- add your account back to your system
- redownload the games/dlc in question
- attempt to do what was causing the issue before
