In a Mace Tyrell game of mine I started with the ambition to fall in love, got the lustful trait by refusing Mace's wife as a candidate and the game suggested that Mace's daughter Margaery becomes his lover. I couldn't refuse and it seemed very game of throney so I rolled with it and Mace impregnated the charming young thing repeatedly.
No one in the world seemed to mind when the two characters were lovers and that they had youngins even if such things are considered pretty foul play-ish in the eyes of gods and men. The kids did get the minus to church vassal relations though.
The only people angry when I legitimized the kids were the wife and sons and Margaery, all for the same reason - because I legitimzed her/our own children.

I realize the game mechanics are a bit flimsy when you dabble into things that aren't in the game's usual framework, such as, err, daddy daughter love, but this could use some work, no? If it were up to me that sort of marriage wouldn't be forbidden but it would give terrible opinion modifiers and bots wouldn't do it unless an event assigns them a family member as lover and they're both unmarried.
 
I got Robert + Myrcella once, but then again I guess that isn't actually incest.

Are you on the current version? I remember that being a known bug but I had thought they fixed it.
