I've got a about 10 Mass Effect themed empires that I've been creating and even though they're all set to forced spawn and I have 10 AI empires set in the settings, I get a lot of random AI empires that aren't the event-based ones like FEs and Maurauders, sometimes only about 3 of the empires I force actually spawn.
 
Many origins are limited to one per galaxy, and forced spawned empires cannot share the same starting solar system (with the exception of the random ones). One of these two things is likely your problem.

How many limits are on Prosperous Unification and On The Shoulders of Giants?
On The Shoulders Of Giants is limited to one per galaxy. No limits on Prosperous Unification. So likely a combination of this + using unique starting systems, maybe?

Weird, because prior to my current save, I've had most of my Mass Effect empires set to Prosperous Unification. It's only in this new save that I figured it would be more fitting to have them all set to On The Shoulders of Giants, because, you know, Mass Effect. All of those empires are also set to Random Unary systems. Anything else that would cause them to be replace with random empires?

Previously, my setup was Krogans with Post-Apocalyptic, Asari with Common Ground (would have most likely included at least one Mass Effect empire, with a small chance of a random), Systems Alliance with Galactic Doorstep, Rachni with Remnants, Quarians with Void Dwellers, and everyone else (Salarians, Geth, Turians, Batarians) with Prosperous Unification. I've created the Hanar with the new Aquatics DLC and given them On The Shoulders of Giants. Any of these conflicting with each other? I've sometimes had less than 5 Mass Effect empires spawn despite having them all on forced and set to 10 AI empires.
 
The only starting systems you can have multiple of are "Random", "Void Dwellers", and "Ocean Paradise" as far as I can tell. (I think multiple "Shattered Ring" can spawn but only one can be forced.)

Any choices other than these are unique and duplicate empires will go into a pool that is selected from on galaxy generation when force spawned. This includes the "Random Unary", "Random Binary", and "Random Trinary" choices.

If that's the case, damn. I'll give it ago next time I start a save, giving the game a bit of a break for now

It works, I've got all my empires in right now. I wonder if all of them will unite and defeat the Reapers.
