I thought it was possible to form the Safavids with Ak Koyunlu, but I reached 1501 and nothing happened (well, besides a couple of events that pretty much ended my game).

you can form it with both of them;however for kara is a bit more difficult depending on one of the monarch's as well if he "survives" or not.

What version do you have? Ak Koyunlu can only form Persia as of the 1.57betas. To do so, you have to pick choice b in "The Question of Sultan Haidar" (1488). Later that year you become the Persians (if you own Azerbaijan), so you either picked the wrong option or you are playing with 1.56.

The Kara Koyunlu can form Persia if they take out AK Koyunlu before 1467 OR if the random number generator decides that Jahan Shah lives.
As I outlined above, the AKK just needs to pick a choice b option, and make sure to have taken Azerbaijan (which doesn't necessarily mean that you've destroyed the Kara Koyunlu, as there is that event that moves the Kara Koyunlu capital to Hamadan).
