I have a fresh Install of the game, original CD, I download the 1.09 patch, install it and start the game. So far everything runs smoothly. When I try to start a scenario (regardless of scenario/country) the game goes down in black, frezes and crashes.

Is this an isolated incident or something more widespread?

Isolated; if it happened to everyone no-one would have ever been able to play the game.

Did the game show version 1.09 as is started up?

Rename the AVI folder under the EU2 folder to, say, AVI_BACK and try running the game again.
 
Okay let's try some standard things.
1) Make sure you have the latest patch for the game.
2) Rename the AVI folder to AVI_back
3) Set the color depth to 16BIT
4) Turn off everything in the background like E-mail/ICQ/Messenger, Virus protection and everything in the system tray beside the clock.
5) Run a Virus check and a Adware/Spybot checker
6) Update your Windows, update your Video card drivers and audio card drivers. Then repeat steps 1-4
 
We had this problem in AGCEEP tech support. It could be because of video drivers. EU2 won't start a scenario with latest ATI drivers but only with "old" version 4.12 ones.

Or, in French Forum, same problem occurred and was entirely tied to read-only files attributes to remove for EU2 folder and all subfolders before applying patch EU2 1.09.

I changed the AVI to AVI_BACK, and now it's working fine.
