The holder=0 line in the landed_title history files (such as Italy) is showing up as a norman character with a black portrait in mods for me.

We have updated our Community Code of Conduct. Please read through the new rules for the forum that are an integral part of Paradox Interactive’s User Agreement.
Looks like that's an issue of the mod(s) rather than of the game itself.
I'm entirely unable to reproduce this within the regular version.
If you can find a situation where this does occur within the unmodded game, that'd be reason to fix it. Alternatively, if it's something that occur PURELY because of a mod being run (and not because of specific scripting in the mod) that would also be a reason to fix it.
But for the moment, I'm going to have to assume that this is an issue specific to those mods you played.
