That would depend on RNG getting the agenda, and I imagine it usually works right, or you would have already been notified and fixed it.

Did you maybe do the rajput agenda "Vassalize U"? That agenda gives the modifier "Foreign Contacts" which gives Diplomatic reputation instead of diplomatic relations. I checked your save and you got that modifier about 1.5 months before the date of the save:
