Looking at the history and how the characters are set up at the start of the game it looks like it should use aganatic primogeniture or a similar law, but it has open elective causing it to pass to unknowns every time which seems wrong.
Ive tried editing it in the save to fix it thinking it was just how it was set up but it defaults back to elective after running the game for a spell as with bishoprics.
The Shia Caliphate works fine and its only the Sunni one that goes all weird. I dont know anything about it historically so maybe its meant to go like that, its just it starts off with a history full of former califs all from the one dynasty and with a string of first sons ready to take their fathers place which gives the impression that it was meant to be working other to how it is.

Alright here is the problem, the first [far left] is a picture of the game loaded at first crusade starting point showing how the history files say it should go.
the second [middle] is the Shia Caliphate's history after running the game, it goes as expected along the line.
the thrid [far right] is the Sunni Caliphate from the same date as the Shia pic, showing that the moment the Calif at the start date dies, it goes to someone random and on likewise from there.

And thats in every single game at every start-date ive played. Maybe thats working as its meant to, but everything, the history, the other Calif, the set up with the son and grand-son ready to go at day one, everything suggests that this is not how its supposed to work. It seems to be running by the same rules as the Pope or Patriarch rather than being in line with the other Calif or its own history files.
The sons never die out, they just get passed over as acting like a bishropic and again, maybe its supposed to, everything about though just gives the impression that this wasnt the intention.
 
I can have another look at it but I doubt that anything will change or that this is such a severe issue that it would need urgent attention. But if what you say is true then it's worth at least knowing and giving another shot.
Last time I tried the Sunnis line of inheritance was of the same house (I killed several rulers to confirm it) but I'll let it play along a little longer then and see if there's a difference.

EDIT: I had another look at it and without killing them "artificially" they still succeeded as primogeniture. Which is quite interesting when it says it's Agnatic Open..

Do you have a save file of it? I can take a look at your game.

I just ran on observe from 1066-96, Calif died twice each time a random, with the son, grandson and greatgrandson all still alive.
I noticed that the Sunni Calif doesnt have an heir circle, while the Shia does, the only other people i could find that dont are the pope and the patriarch. Hope it helps, if no one else has the problem i'll just try re-installing.
