Is it even possible to marry your heir to a duchess/countess? In all my years of CK I've never been able to achieve this.

It is possible, but highly unlikely
... you need to be very prestigous for the AI female rulers to even consider your heir. But I have heard of people who managed it (without cheating .. or so they say)
