Seems kinda ridiculous I have to choose between murdering my wife because she had a miscarriage year ago or eat a ton of stress. Character is zealous, paranoid, and vengeful on top of having irritable, so maybe that affects it.

Here's the tl;dnr
Yes. CK3 only considers if a memory is good or bad. It noticed you have a bad memory of a miscarriage and gave you an event to get revenge on someone involved in your bad memory.

It is a real event trigger, created by Daniel Moore. It's part of the so called "Dan Events". This is Dan's Twitter: https://x.com/daniel_moore22

The event can be found in bp1_dan_events.txt. Here's what causes it:
  • It has a yearly chance to trigger, but has a cooldown of 10 years.
  • The player must be at peace
  • The player has Friends & Foes dlc
  • The player has a negative memory, and someone who participated in that memory is alive but is NOT you, nor a friend or lover of you.
  • Said character is in diplomatic range and is a valid target to start a murder scheme (e.g.: not your child)
  • You are not forgiving.
You will lose stress for starting the murder scheme because you are vengeful. However, it's worth mentioning that you can just cancel the scheme as soon as it starts.
You are correct, you are gaining major stress for forgiving her because you are vengeful. Paranoid is DOUBLING that stress gain, too

Pretty sure she feel bad about it too.

Unless she went to see a wise woman for something to stop the pregnancy, possibly because it wasn't her husband's. XD
 
Classic Friends & Foes stuff right there. But there's not enough toilet humour in this event. That was the trend back then for CK3 humour, besides absolutely low effort triggers.
