The event can be found in bp1_dan_events.txt. Here's what causes it:
- It has a yearly chance to trigger, but has a cooldown of 10 years.
- The player must be at peace
- The player has Friends & Foes dlc
- The player has a negative memory, and someone who participated in that memory is alive but is NOT you, nor a friend or lover of you.
- Said character is in diplomatic range and is a valid target to start a murder scheme (e.g.: not your child)
- You are not forgiving.

I strongly suggest you report this as a bug. It seems pretty clear to me that this is an oversight and not an intended result of the event.

Classic Friends & Foes stuff right there. But there's not enough toilet humour in this event. That was the trend back then for CK3 humour, besides absolutely low effort triggers.

I cunningly avoid events like this by simply never buying Friends and Foes.
