I'm getting the same problem as others. I have Vista 32 SP1. I have patched to 1.1. My video and audio drivers are up to date. I have tried Administrator and compatibility mode and still get the crash.

does running in window mode help?
 
Ah, you have two graphics adapters, which may be a problem. But try changing the refreshRate in settings.txt from 0 to 60.

Well, that got me to the main menu. Now when I try to run a tutorial or a single player game it crashes when 'adapting history' shows up.
 
Alright well 60 is just a common number, it may not be exactly right for the resolution you are using. Change your desktop resolution to 1024x768 (ie the same as what you are using in the game) then run DXDIAG again, you can see what refresh rate that resolution uses on your system, and put that in settings.txt .
