So I'm enjoying my time in this game, chilling in the year 1230 or so as a pretty powerful duchess. I have Transylvania, Nystria or however it's spelled, and some other area, but I also have this claim on my cousin, the duchess of Wallachia.

Well, my dear cousin has a pretty damn large number of levies she can raise (when our grandfather had ruled both our areas before gavelkind succession had fucked over the unified claim, he'd built up the already fairly prosperous area) but I have a pretty damn large number of gold coins, so I recruit a couple mercenaries for the war. And the war goes as expected, cousin crushed, yada yada yada. But then I disband my mercenaries, and too my shock and disappointment, I keep paying them. I've sent the companies away, but I'm still giving both of them forty gold each month.

I'm pretty sure I'm up on my patches, and haven't seen this anywhere else in the forum (maybe I wasn't looking hard enough?)

Mercenaries can come with boats too, did you disband those?

Or alternatively, are you relying on the tool tip over your money balance to know how much you're paying? That tends to be delayed in updating (usually by a week or so). If you check the Military screen you'll see the actual amount of upkeep your various kinds of military are costing you.
